Social Worker
St. James Rehabilitation and Healthcare Center is seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. This is an exciting opportunity to make a meaningful difference in the lives of our residents by providing emotional support, advocacy, and care coordination in a skilled nursing environment.

Conduct resident assessments to identify social, emotional, and psychosocial needs
Develop, implement, and monitor individualized care plans in collaboration with the healthcare team
Provide counseling, advocacy, and support to residents and their families
Assist with discharge planning, ensuring safe and appropriate transitions to home or other care settings
Collaborate with interdisciplinary team members to promote resident-centered care
Maintain accurate documentation and records in compliance with state and federal regulations
Serve as a resource for community services and benefits for residents and families
Bachelor’s or Master’s degree in Social Work (BSW or MSW) from an accredited program
Valid New York State Social Work license (LMSW or LCSW preferred)
Experience in skilled nursing or long-term care preferred
Strong communication, organizational, and interpersonal skills
Ability to work collaboratively as part of a multidisciplinary team
Compassionate, proactive, and resident-focused
