Duties
- Conduct thorough patient assessments, including diagnostic evaluations and mental health screenings, to determine individual needs and treatment plans.
- Develop and implement personalized care plans that incorporate behavioral health strategies, medical management, and community resource coordination.
- Manage clinical case loads by prioritizing tasks effectively, ensuring timely interventions and follow-up care.
- Collaborate with physicians, social workers, community agencies, and families to coordinate services and facilitate smooth discharge planning.
- Maintain accurate documentation of patient interactions, treatment progress, and medical records using Electronic Health Records (EHR) systems in compliance with HIPAA regulations.
- Provide mental health counseling and addiction counseling tailored to patients with substance use disorders or behavioral health conditions.
- Participate in interdisciplinary team meetings to review patient progress, adjust care plans as necessary, and advocate for patient needs across healthcare settings including inpatient, outpatient, ICU, PICU (Pediatric Intensive Care Unit), and post-acute care environments.
Requirements
- Bachelor's or Master’s degree in Social Work, Psychology, Counseling or related field; licensure or certification preferred.
- Proven experience working within hospital settings such as Level I Trauma Centers or Level II trauma centers; ICU or emergency medicine experience highly desirable.
- Strong knowledge of medical terminology, physiology, diagnostic evaluation techniques, and clinical case management principles.
- Experience working with children, individuals with disabilities, mental health conditions, or substance use disorders is essential.
- Proficiency in Electronic Health Records (EHR) management systems; familiarity with HIPAA regulations is required.
- Excellent client communication skills with the ability to work effectively with diverse cultural backgrounds and family units.
- Ability to prioritize tasks efficiently while maintaining high-quality patient service and care standards.
- Relevant certifications such as Basic Life Support (BLS), CPR, or Managed Care credentials are advantageous.

Pay: $24.84 - $29.92 per hour
