Overview
WellCare Support Services, a care agency operating under the New Jersey Division of Developmental Disabilities (DDD), is seeking the services of a Licensed Practical Nurse (LPN) to provide nursing support for individuals with developmental disabilities in the Newark, New Jersey area.
The ideal candidate will possess a current New Jersey LPN license, demonstrate strong clinical and communication skills, and have a commitment to delivering high-quality, person-centered care. Experience working with individuals with developmental disabilities is preferred.

Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
- Administer medications and injections accurately following physician orders and safety protocols, including IV infusions and medication administration techniques.
- Assist with diagnostic evaluations, specimen collection and processing, and basic medical imaging procedures to support clinical decision-making.
- Manage patient care plans by coordinating with multidisciplinary teams, including case management for complex cases involving dementia, Alzheimer's disease, or memory care.
- Provide specialized care such as wound dressing changes, catheterization, tube feeding, ventilator management, and sterile technique procedures.
- Support patients in acute pain management, hospice care, or post-acute settings while adhering to HIPAA regulations and maintaining meticulous medical documentation.
- Educate patients and families on health coaching topics like medication adherence, nutrition, discharge planning, and chronic disease management.
- Ensure infection control standards are maintained through aseptic techniques and proper sterilization practices in all clinical activities.
Requirements
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice in your jurisdiction.
- Proven experience working in diverse healthcare environments such as hospitals (including ICU or Level I Trauma Center), nursing homes, assisted living facilities, or outpatient clinics.
- Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/ICD-10), CPT coding, and electronic medical records systems like Epic or Athenahealth.
- Experience with specialized areas including critical care (ICU), emergency medicine (ER), geriatrics, pediatrics, or behavioral health is highly desirable.
- Ability to perform technical procedures such as phlebotomy, venipuncture, catheter care, suturing assistance, and airway management.
- Excellent patient assessment skills combined with compassionate communication tailored to individuals with disabilities or cognitive impairments like dementia or Alzheimer’s disease.
- Physical ability to stand for extended periods and perform lifting or repositioning tasks using equipment such as Hoyer lifts when necessary. Pay: $28.00 - $30.00 per hour
