LTC Therapy is seeking an Occupational Therapist (OT) to join our rehabilitation team in a skilled nursing facility setting. This position offers the opportunity to create meaningful, individualized treatment programs that help residents improve safety, independence, and participation in everyday life.
What You'll Focus On:

Complete comprehensive occupational therapy evaluations and identify functional limitations impacting daily routines

Develop personalized treatment strategies targeting self-care, cognition, upper extremity function, balance, and adaptive techniques

Train residents in the use of adaptive equipment and compensatory strategies to improve independence and reduce fall risk

Collaborate with nursing, rehab staff, physicians, and families to support continuity of care and successful outcomes

Monitor resident performance, update care plans, and maintain compliant documentation

Qualifications:

Active Occupational Therapist license required

Strong communication and clinical reasoning skills

Skilled nursing or geriatric experience preferred
