Position Summary:
The Registered Nurse (RN) plays a vital role in delivering skilled nursing care to clients in their homes. This position involves comprehensive assessments, care planning, clinical interventions, and coordination with interdisciplinary team members to ensure high-quality, client-centered care.
Key Responsibilities:

Conduct initial and ongoing comprehensive assessments of clients’ physical, emotional, and environmental needs.

Develop, implement, and revise individualized care plans in collaboration with clients, families, and care teams.

Provide skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.

Educate clients and caregivers on health conditions, medications, and self-care techniques.

Monitor and document client progress, response to treatment, and changes in condition.

Communicate effectively with physicians, therapists, social workers, and other care providers.

Supervise and support home health aides and licensed practical nurses (LPNs) as needed.

Ensure compliance with agency policies, state regulations, and federal guidelines.

Participate in case conferences, quality improvement initiatives, and continuing education.
Qualifications:

Current and unrestricted RN license in the state of employment.

Minimum of one year of clinical nursing experience (home health experience preferred).

CPR certification (BLS required; ACLS preferred).

Valid driver’s license and reliable transportation.

Strong clinical judgment, time management, and communication skills.

Proficiency in electronic health records (EHR) systems.
