Life Skills Trainer
Mi Casa Transitional Services, Inc. provides services through the State of Colorado Transitions Program to Medicaid members living in long-term care facilities who are transitioning back into the community.
We are seeking dependable, compassionate, and self-motivated Life Skills Trainers to work directly with individuals in their homes and community settings. This is an independent, community-based position that requires travel between client locations throughout the workday.
POSITION DETAILS
Starting pay: $20.00 per hour, commensurate with experience
Job type: Part-time with the potential to become full-time
Schedule: Flexible
Locations: Colorado Springs and surrounding service areas, including Pueblo and Cañon City
Health insurance: Available to eligible full-time employees.
RESPONSIBILITIES
Life Skills Trainers assist clients with developing and maintaining the skills necessary to live as independently as possible in the community.
Responsibilities may include:
- Providing individualized life-skills training based on each client's needs and goals
- Assisting clients with household organization, cleaning, meal planning, and preparation
- Teaching budgeting, shopping, and other money-management skills
- Assisting clients with accessing community resources and services
- Helping clients develop routines and improve independent living skills
- Providing transportation-related training and assistance within the community as appropriate
- Attending appointments or community activities with clients when required
- Maintaining accurate and timely documentation of services provided
- Communicating professionally with clients, families, service providers, and Mi Casa Transitional Services, Inc.
- Following client service plans, company policies, and applicable program requirements
QUALIFICATIONS
- High school diploma or equivalent
- At least one year of Human Services experience preferred
- Valid driver's license
- Reliable transportation
- Ability and willingness to travel independently between clients' homes and other community locations throughout the workday
- Ability to work independently and manage time effectively
- Dependable, professional, and comfortable working directly with individuals in their homes and community
- Ability to complete required documentation accurately and on time
- Ability to pass a required background check
ADDITIONAL INFORMATION
This position begins as part-time with the possibility of becoming full-time based on client needs, availability, and job performance.
Because services are provided in clients' homes and throughout the community, applicants must have reliable transportation and be willing and able to travel between client locations as required.
