Do you have a penchant for organizing, collaborating, smiling, and having fun? We are a medical stabilization unit located in a prominent Dallas hospital. Our team provides support and medical care to those patients undergoing alcohol and drug withdrawals, among other medical complications.
We are currently seeking a Discharge Coordinator / Discharge Planner. The role involves continuous assessment of the patient's status and thorough documentation of pertinent information. It also requires accurate documentation of communication and coordination for the patient's discharge and transition to the next level of care. The incumbent will serve as an information resource to patients and maintain consistent and effective communication with all departments within the unit. Additionally, they will be responsible for matching resources and referrals to meet the needs of the patients.
Licenses, Registrations, Certifications, or Special Requirements:
Valid Texas Driver's License.
*One year or more of experience in discharge planning or supporting patients transitional care is preferred (but not required)
* Individuals in recovery with at least one year of sobriety are strongly encouraged to apply
Job Type: Full-time
Pay: $45,000.00 - $55,000.00 per year
