Position Summary
The RN provides skilled nursing care to patients in their homes, performs assessments, develops care plans, coordinates services with physicians and therapists, and helps patients achieve the highest level of independence and health.
Responsibilities
- Perform patient assessments and nursing visits
- Develop and update individualized care plans
- Monitor patient conditions and communicate changes to physicians
- Educate patients and caregivers
- Coordinate care with interdisciplinary team members
- Complete clinical documentation accurately and timely
- Conduct supervisory visits as required
- Ensure compliance with agency, state, and federal regulations
Qualifications
Required:
- Active Ohio RN License
- Current CPR Certification
- Valid Driver’s License and Reliable Transportation
- Strong Clinical Assessment Skills
- Excellent Communication and Documentation Skills
Preferred:
- Home Health Experience
- Medicare Home Health Experience
- OASIS Experience
- Case Management Experience
New to Home Health but an Experienced RN?
