TruCare Home Health is seeking experienced, compassionate, and motivated Weekend and PRN Registered Nurses (RNs) to provide exceptional skilled nursing care to patients in their homes. This position is ideal for an RN with strong clinical assessment skills, excellent time management, and prior Homecare Homebase (HCHB) experience.

Position Summary
The Home Health Registered Nurse provides comprehensive skilled nursing services to patients in accordance with physician orders, agency policies, Medicare Conditions of Participation, and state regulations. The RN is responsible for completing patient assessments, OASIS documentation, care planning, patient and caregiver education, coordination of care, and timely clinical documentation.
Responsibilities
- Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, Routine, Supervisory, and Discharge visits.
- Complete accurate and timely OASIS assessments.
- Develop, implement, and update individualized patient care plans.
- Perform comprehensive head-to-toe nursing assessments.
- Provide skilled nursing interventions according to physician orders.
- Educate patients and caregivers regarding disease management, medications, safety, and self-care.
- Monitor patient progress and communicate significant changes to physicians and the interdisciplinary team.
- Coordinate care with therapists, social workers, physicians, and office staff.
- Complete documentation within required agency timeframes using Homecare Homebase (HCHB).
- Participate in case conferences and quality improvement initiatives.
- Maintain compliance with Medicare, state, and agency regulations.
QualificationsRequired
- Current unrestricted Registered Nurse (RN) license.
- Minimum one year of recent Home Health nursing experience.
- Previous Homecare Homebase (HCHB) experience is preferred.
- Experience with Medicare home health documentation and OASIS.
- Current BLS certification.
- Valid driver's license, reliable transportation, and current automobile insurance.
- Ability to work independently while managing multiple patient visits.
- Excellent communication, organizational, and critical thinking skills.
Preferred
- Experience with wound care, IV therapy, cardiac care, diabetes management, and chronic disease education.
- Knowledge of PDGM and Medicare home health regulations.

- Weekend Registered Nurse
- Saturday and Sunday availability required
- Flexible scheduling available
- PRN Registered Nurse
- As-needed weekday and/or weekend visits
- Flexible scheduling based on patient census
