Weekend Track RN - Community Hospice Admissions Team
The Community Hospice is seeking a compassionate and motivated Registered Nurse (RN) to join our Weekend Hospice Admissions Team. This rewarding role focuses primarily on admissions to our Hospice Home Care program, with cross-training opportunities to support admissions across other levels of hospice care.
Workdays only Every Saturday&Sunday 8a-8p - no hours required during the week once trained

What you will do:

The Hospice Admissions RN is responsible for evaluating and admitting patients to hospice services in the home setting while ensuring a smooth transition into care. This role combines clinical expertise, compassionate communication, and coordination with physicians, patients, families, and the hospice interdisciplinary team.
Key Responsibilities

Conduct hospice admissions for patients in their homes, averaging two home admissions per day.

Collaborate with community physicians and Hospice Medical Directors to confirm hospice eligibility.

Complete comprehensive assessments of patient and family needs.

Initiate the plan of care, including coordination of: Medications, Durable medical equipment, Medical supplies, Interdisciplinary support services

Coordinate care with social workers, chaplains, volunteers, complementary therapists, home health aides, and other team members.

Ensure a seamless handoff to the assigned Hospice Case Manager.

Accurately document assessments, care plans, and admission-related information
Service Area & Travel:
The Community Hospice's corporate office is located at 445 New Karner Road, Albany, NY, with satellite offices in Catskill and Saratoga Springs, NY. Admissions are completed throughout our eight-county service area: Albany, Columbia, Greene, Rensselaer, Saratoga, Schenectady, Washington, Montgomery.
Patient assignments are made with consideration for your geographic region whenever possible.
What you will need:

Valid NYS RN license

Associate’s degree in nursing. BSN Preferred

1 year of RN experience is required. At least 1 year experience as an RN working in homecare highly preferrable.

Previous hospice experience preferred but NOT required.

Valid driver license in good standing and reliable vehicle

Strong communication and interpersonal skills.

Ability to document clinical information concisely and accurately.

Strong computer proficiency.

Excellent organizational and time management skills.

Must be able to communicate effectively with patients, physicians, and peers.

SUPPORTED BY A HOSPICE RN SUPERVISOR in the office 8am to 4pm Saturday and Sunday and an OnCall RN Supervisor after hours, so assistance and guidance is only a phone call away!
WEEKEND TRACK RATE: $57.90
Experience based (non-weekend track) Pay Range:$38.00 - $50.18
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
