We are seeking a highly skilled and dedicated SCN Intake Coordinator with Arabic fluency to join our team in Suffern, NY. This role will support members of the Arabic-speaking and Muslim community by assisting individuals and families through the SCN enrollment process, identifying their needs, and connecting them with available healthcare and community resources.
As a key member of our intake team, you will serve as a primary point of contact for clients and families, providing compassionate support, gathering required information, and ensuring a smooth and efficient intake process. The ideal candidate is fluent in Arabic, has excellent communication skills, and is passionate about helping individuals access the services and support they need.
Intake Coordination
Conduct intake interviews, screenings, and eligibility assessments for new clients.
Collect and document required demographic, insurance, medical, and social needs information.
Ensure accurate and timely completion of intake documentation.
Communication & Client Support
Communicate with clients and families in Arabic and English to gather information, answer questions, and provide guidance throughout the enrollment process.
Build positive relationships with members of the Arabic-speaking and Muslim community.
Provide education regarding available SCN services, benefits, and resources.
Service Coordination
Connect clients with available resources and supports, including healthcare services, community programs, and approved SCN benefits.
Collaborate with care managers, healthcare providers, therapists, and community partners to support client needs.
Follow up with clients to ensure successful enrollment and continued engagement.
Documentation & Compliance
Maintain accurate records in required systems.
Ensure compliance with SCN, Medicaid, HIPAA, and company policies.
Maintain confidentiality of client information.
Fluent in Arabic, both written and spoken.
Strong communication and relationship-building skills.
Excellent organizational and time management skills.
Ability to manage multiple priorities while maintaining attention to detail.
Experience working with healthcare, social services, or community-based programs preferred.
Ability to work effectively with diverse populations and multidisciplinary teams.
