POSITION SUMMARY:
The Administrative Coordinator is a part-time administrative and data-focused position responsible for supporting program operations, participant tracking, and outcomes monitoring for the MAT Reentry and Outcomes Tracking Initiative in Bucks County. This position plays a critical role in ensuring the accuracy, integrity, and timeliness of all program data related to participants transitioning from the MAT Program in the Bucks County Correctional Facility to community-based care. A critical part of this work is conducting follow up contacts with participants and treatment providers at regular intervals following release from incarceration up to one year.
The Support and Outcomes Coordinator will work closely with program leadership, the Human Services Reentry Navigator, Hub Coordinator, case managers, BCDOC and PrimeCare staff, and community partners to maintain tracking systems, support participant engagement efforts, and produce data reports that inform program performance, compliance, and continuous quality improvement with an overall goal of improving outcomes and reducing recidivism.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Accurately enter, update, and maintain participant data in a secure case management system

Collect, enter, update and audit MAT participant-level data

Track and monitor key performance indicators, including: o MAT linkage and retention

Appointment attendance and service engagement

Overdose incidents and critical events
-
Recidivism indicators (rearrest, violations, reincarceration)

Monitor participant progress across all required tracking intervals

Generate routine reports for program leadership and funders

Assist in analyzing data trends and identifying areas for improvement, including any gaps in service delivery

Collaborate with the evaluation team to develop data collection methodologies and to analyze data and identify trends

Conduct outreach to participants to support follow-up data collection and engagement

Identify engagement barriers and notify internal partners or supervisors

Provide appointment reminders and basic navigational support

Provide support and oversight to participants using the mobile Reentry App

Assist in verifying participant engagement in services through provider contact
QUALIFICATIONS REQUIRED:

Associate’s or Bachelor’s degree in public health, human services, criminal justice, or related field preferred, or equivalent experience

1–2 years of experience in administrative support, data entry, or program coordination

Experience with case management or data systems preferred

Experience with justice-involved or behavioral health populations preferred

Proficiency in Microsoft Office (especially Excel)

Strong organizational and time management skills

Effective communication and coordination abilities

Cultural competency and trauma-informed approach
