Meadow Park Rehabilitation & Healthcare Center is seeking a dedicated and compassionate Full-Time Physical Therapist to join our rehabilitation team. This is an excellent opportunity for a motivated therapist to make a meaningful impact while working in a collaborative and resident-focused environment.
Responsibilities
- Evaluate residents and develop individualized treatment plans.
- Provide skilled physical therapy services to short-term rehabilitation and long-term care residents.
- Monitor resident progress and modify treatment plans as appropriate.
- Collaborate with physicians, nursing staff, and interdisciplinary team members.
- Maintain timely and accurate documentation in accordance with facility and regulatory requirements.
- Educate residents and families regarding treatment goals and therapeutic programs.
Qualifications
- Current New York State Physical Therapist license in good standing.
- Strong clinical, communication, and organizational skills.
- Skilled nursing facility experience preferred, but not required.
