What will I do in this role with Judson?
-
Monitors care delivery and makes changes as needed to care plan for home care clients.

Maintains required documentation including developing, implementing and evaluating the care plan, as changes in condition occur and as required.

Oversight of care delivered by the home care aide and makes adjustments to plan of care as needed. Coaches the home care aid to improve care delivery.

Monitors resident/community client appropriateness in current level of care, assesses and advises when other services may be appropriate.

Delivers and coordinates case management services; and making referrals for other services accordingly.

Effective communication with residents and families. Actively listens to resident and family concerns and identifies solutions.

Participates in Quality Improvement initiatives.

Initiates and participates in care conferences and maintains effective communication with interdisciplinary teams.

Makes home visits to prospective move-ins – for assessment and alignment with appropriate level of care.

Completes intake, assessments and development of plan of care for home care clients.

Hospice duties:
-
Assures that hospice documentation is completed in accordance with Medicare rules and regulations and within the specified time frames.

Delivers hospice service in accordance with Medicare regulations and mandates and maintains the comfort and quality of life for those who are diagnosed with terminal diseases.

Effective communication with families. Actively listens to family concerns and identifies solutions.
What do I need for this role with Judson?

License Practical Nurse

Previous experience in hospice

Knowledge of hospice Quality standards and Conditions of Participation.

Ability to routinely travel locally and periodically work a non-traditional schedule/outside regular business hours if needed
