Position Summary: Women's Health CT is currently seeking an experienced Nurse Practitioner (APRN) for our Gestational Diabetes Management Program. The Nurse Practitioner will provide expert, patient focused education and care of individuals diagnosed with Gestational Diabetes via a Telehealth platform and collaborate closely with MFM and OB/GYN physicians, and collaborative care providers in a virtual setting.
In collaboration with the lead provider, this role is responsible for providing skilled health care to patients via telehealth, facilitating clinical management of patients by collaborating with OB providers, clinical nurses and other members of health care team.
Summary of Duties and Responsibilities:
- Educate and monitor patients with gestational diabetes via a telehealth platform and provide counseling to patients regarding expected course of treatment.
- Obtains and documents health history and orders and interprets diagnostic tests.
- Collaborates with physicians to provide appropriate direct care interventions within scope of practice.
- Collects, organizes, records, and discusses data as appropriate and uses sound clinical judgment based in best practice standards and evidenced based medicine.
- Prescribes pharmaceuticals according to patient needs and conducts appropriate patient follow-up and outreach.
- Demonstrates commitment to ongoing education and improvement as evidenced by documentation of attendance at conferences, in-services, seminars, and committee meetings.
- Educate patients on the use of continuous glucose monitor and insulin injection administration when indicated.
- Performs other related duties in support of the Gestational Diabetes Management Program, including, but not limited to, scheduling and billing.
Qualifications and Requirements:
- Licenses and Certifications: Required current CT Nurse Practitioner (APRN) License with prescriptive authority. National Certification in Advanced Practice. Training or certification in Adult Health, Family Health or related primary care licensure. Diabetes Educator Certification is preferred.
- At least 2 years of experience in primary care, emergency, or urgent care preferred
- Experience working with patients with gestational diabetes
- Customer-Focus and Communication
- Excellent written and verbal communication skills.
- Ability to communicate in English. Spanish is highly preferred
- Positive, customer-focused approach, with commitment to providing excellent patient care.
- Excellent clinical management skills and proven ability to work collaboratively in a team environment and take lead when appropriate
- Highly organized with demonstrated ability to effectively prioritize competing tasks with full autonomy
- Knowledge of computer systems, including experience with electronic medical record system (Athena)
- Excellent clinical management skills and proven ability to work collaboratively in a team environment and take lead when appropriate
- Ability to adhere to PWH policies as well as HIPAA requirements, OSHA regulations and Infection Control standards

Pay: $60.00 - $65.00 per hour
