Responsibilities
Monitor and document the client's condition throughout the night.
- Obtain and record vital signs as ordered.
- Administer medications within the LVN scope of practice (if applicable).
- Assist with personal care, repositioning, and incontinence care.
- Monitor oxygen use and report any changes in condition.
- Maintain accurate documentation and communicate concerns promptly.
- Provide compassionate, respectful, and professional care.
- Collaborate with the client's family and agency staff to ensure continuity of care.
Qualifications
- Current Texas LVN license in good standing.
- Current CPR/BLS certification.
- Private duty, home health, hospice, or long-term care experience preferred.
- Excellent assessment, communication, and documentation skills.
- Reliable transportation.
- Ability to work independently and maintain professionalism at all times.

- Setting: Private Duty – One Client in the Home
- Start Date: Immediate.
