You'll help our physician team provide primary and urgent care to complex home bound patients.
YOUR ROLE
- You'll help join a physician led clinical team bringing value based care to complex home bound patients. Our physician practice is focused on helping patients age at home while avoiding the hospital and SNF.
- Must be comfortable with patient assessments including gathering history, medication reconciliation, symptom and patient request triage.
- Must be comfortable communicating with provider partners e.g., Home Health clinicians, PCP offices, Subspecialty offices, case managers
Requirements
- Must have healthcare credential e.g., Community Health Worker, SW, RD, EMT, RN, MPH, MHA, MA or other healthcare qualification
- At least 3 years of experience and exposure to home based care preferred
- Ideal candidate is self starter, detailed oriented and comfortable with modern software
- This is an in-person role however there is no in-person patient contact

Pay: $22.00 - $26.00 per hour
