Job Summary
We are seeking a dynamic and compassionate Nurse Practitioner to join our remote patient care team, specializing in Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Real-Time Monitoring (RTM), and Behavioral Health Integration (BHI). Responsibilities
- Conduct comprehensive virtual patient assessments, including medication reconciliation, and symptom review, utilizing advanced telehealth tools and EMR systems such as Epic, eClinicalWorks, or Athenahealth.
- Develop personalized care plans for patients with chronic conditions such as diabetes, hypertension, heart failure, or behavioral health issues, ensuring alignment with best practices and clinical guidelines.
- Monitor patient progress through RPM devices and RTM platforms, analyzing data to identify early warning signs and intervene proactively to prevent hospitalizations or complications.
- Collaborate with multidisciplinary teams—including physicians, specialists, and case managers—to coordinate care delivery and optimize treatment outcomes.
- Provide education on disease management, medication adherence, lifestyle modifications, and self-care strategies tailored to individual patient needs.
- Document all patient interactions accurately in electronic health records while maintaining compliance with HIPAA regulations and documentation standards.
- Participate in quality improvement initiatives, clinical research projects, and ongoing training to enhance telehealth services and clinical workflows.
Qualifications
- Current Nurse Practitioner licensure in good standing with state licensing boards; certification in family practice or adult-gerontology preferred.
- Strong experience in remote patient care, including CCM, RPM, RTM, and BHI programs.
- Proven ability to create and update care plans for chronic care and remote monitoring patients.
- Experience with remote care management software platforms (HealthArc, Prevounce, or similar).
- Excellent remote patient communication and engagement skills.
- Familiarity with CPT coding for billing purposes; knowledge of Workers’ Compensation law and utilization review processes is a plus.
- Excellent communication skills with the ability to educate patients effectively while demonstrating empathy and cultural competence.
