About the role
Perma Homecare is a growing homecare agency dedicated to providing compassionate, high-quality care to our clients across the greater Cleveland area. We are looking for a motivated, organized, and hands-on professional to join our team as an HR Coordinator / Direct Care Relief Worker. This is a unique dual-role position — you'll support the office with human resources functions while also stepping into the field to provide direct care coverage when a caregiver calls off.
What you'll do
HR & Office duties
- Recruit, screen, and onboard new direct care staff
- Maintain employee files, certifications, and compliance records
- Coordinate background checks and reference verifications
- Track employee attendance, call-offs, and scheduling gaps
- Assist with payroll preparation and timekeeping accuracy
- Ensure ongoing compliance with Ohio ODA / PASSPORT requirements
- Support staff with HR questions, disciplinary documentation, and performance tracking
Field relief duties (as needed)
- Provide hands-on personal care and homemaker services when caregivers call off
- Assist clients with activities of daily living (ADLs) including bathing, grooming, meal prep, and light housekeeping
- Document visit notes accurately in compliance with agency and payor requirements
- Maintain a professional, caring approach in all client interactions
What we're looking for:
Required
- Valid Ohio STNA or home health aide certification (or willingness to obtain)
- 1+ year of HR, office admin, or home health experience
- Strong organizational and communication skills
- Valid Ohio driver's license and reliable transportation
- Able to pass background check (BCI/FBI)
Preferred
- Familiarity with PASSPORT / Ohio Medicaid home care
- Experience with EVV (electronic visit verification) systems
- Prior HR or recruiting experience in a healthcare setting
- Bilingual a plus
