Memory Care Unit Supervisor

The Memory Care Unit Supervisor is responsible for overseeing the psychosocial well-being, behavioral support, and individualized care planning of residents within the memory care unit. This role works closely with the interdisciplinary team to ensure residents receive compassionate, person-centered care that supports their emotional, cognitive, and social needs. The Memory Care Unit Supervisor will also oversee the development and implementation of meaningful therapeutic activities and ensure activity staff are appropriately carrying out individualized programming for residents.
Job Duties
Coordinates and participates in resident assessments, care planning, and care conferences for memory care residents.
Develops, updates, and maintains individualized care plans reflecting residents' psychosocial, behavioral, emotional, and activity needs.
Completes psychosocial assessments, social service documentation, behavioral monitoring, and discharge planning as appropriate.
Assists with PASRR-related documentation, compliance, and coordination of services. Participates in interdisciplinary team meetings and collaborates with nursing, therapy, activities, dietary, and administration to support resident-centered care.
Supports residents and families with adjustment to placement, behavioral changes, emotional concerns, and transitions of care.
Maintains regular communication with family members, responsible parties, and healthcare representatives regarding resident status, goals, and discharge planning.
Advocates for resident rights, dignity, safety, and quality of life.
Assists in resolution of grievances or concerns voiced by residents, family members, responsible parties, or ombudsman.
Provides guidance and support to staff regarding dementia care approaches, communication techniques, and resident-specific interventions.
Oversees the development of individualized therapeutic activity programs for memory care residents.
Works alongside the Activities Director to coordinate group, individual, bedside, seasonal, and special event programming.
Monitors activity participation, documentation, and resident engagement to ensure quality and compliance standards are met.

Requirements
High School Diploma required
Previous Skilled Nursing Facility experience preferred
Experience with dementia or memory care preferred
Knowledge of care planning, care conferences, psychosocial assessments, and PASRR processes preferred
Some prior nursing home activities experience preferred
Qualified social worker with at least one (1) year of supervised social work experience in a healthcare setting working directly with individuals
Acceptable social work backgrounds include:
Social Services Worker (SSW)
Certified Social Worker (CSW)
Licensed Clinical Social Worker (LCSW)
