1. Expand, enhance and improve a diverse selection of wellness and prevention services to beneficiaries and service recipients of TCC Health Services, in alignment with the TCC Health Services Vision, Mission and Strategic Plan.
2. Work collaboratively with division directors of Clinical Services to evaluate needs, design, and implement clinically related wellness and prevention services in support of these divisions.
3. Identify and develop programs/activities that promote healthy lifestyles and support behavioral changes that will contribute towards the health services vision “healthy people across generations” and will assist patients in improving health and wellness outcomes.
4. Assess, plan, develop, implement, monitor and evaluate culturally relevant programs to promote holistic wellness and prevention programming in order to assist patients in improving health and wellness outcomes, through health promotion, disease prevention, clinical preventive services, and behavioral health outreach and prevention services, including suicide prevention.
5. Provide input for public health and development of community strategies and local strategic partnerships; support community efforts to promote healthy communities.
6. Work closely with and provide strategic support to the Reclaim our People initiative (war on alcohol and drugs movement), in supporting communities to address substance abuse and the impact of substance abuse at a community level.
7. Promote and utilize evidence based guidelines at all stages of program development and implementation.
8. Participate in quality improvement process and customer satisfaction initiatives.
9. Perform statistical reporting, evaluation and analysis of programs in collaboration with the Quality Management department.
10. Develops relationships with external stakeholders and agencies to enhance and support healthy lifestyle behaviors.
