Position Summary
Teamwork Therapies is seeking a qualified Physical Therapist (PT) to provide school-based physical therapy services to students in the Concord, California area. The Physical Therapist will work collaboratively with students, families, teachers, special education staff, administrators, and other related-service professionals to support students’ access, mobility, positioning, safety, and participation within the educational environment.
The School-Based Physical Therapist will evaluate students, develop individualized treatment recommendations, provide direct and consultative physical therapy services, participate in IEP meetings, and collaborate with school teams to support students in achieving functional educational goals.
Primary Responsibilities
- Conduct comprehensive school-based physical therapy evaluations and re-evaluations for students referred for assessment.
- Review student records, medical history, prior evaluations, IEPs, teacher input, and other relevant information.
- Assess gross-motor skills, mobility, balance, strength, coordination, posture, positioning, transfers, endurance, and functional participation within the school environment.
- Evaluate a student’s ability to safely access classrooms, playgrounds, restrooms, transportation, and other school environments.
- Develop measurable, educationally relevant IEP goals and objectives based on evaluation findings and student needs.
- Provide direct physical therapy services in accordance with each student’s IEP.
- Provide consultative services to teachers, paraprofessionals, families, and other school personnel.
- Recommend strategies, positioning techniques, adaptive equipment, mobility supports, and environmental modifications as appropriate.
- Support students with transfers, gait, mobility, wheelchair positioning, and safe participation in school activities.
- Collaborate with Occupational Therapists, Speech-Language Pathologists, Special Education teachers, Adaptive Physical Education staff, nurses, psychologists, and other members of the multidisciplinary team.
- Participate in IEP meetings, assessments, annual reviews, triennials, and transition planning as appropriate.
- Present evaluation findings, progress, recommendations, and proposed goals during IEP meetings.
- Monitor student progress toward IEP goals and modify intervention strategies as appropriate.
- Complete progress reports and other required documentation within established school and regulatory timelines.
- Maintain accurate treatment notes, assessment records, attendance documentation, and service logs.
- Provide training and education to school staff regarding safe transfers, positioning, mobility, equipment use, and other PT-related student needs.
- Assist with selection and implementation of adaptive equipment and mobility devices when appropriate.
- Communicate with parents and caregivers regarding student progress and strategies that may support functional participation.
- Maintain confidentiality of student and family information in accordance with FERPA, HIPAA when applicable, and professional standards.
- Follow school district policies, Teamwork Therapies procedures, and applicable state and federal special education requirements.
- Maintain a safe environment during all therapy and mobility activities.
Qualifications
- Graduate degree from an accredited Physical Therapy program.
- Current and unrestricted California Physical Therapist license.
- Current CPR/BLS certification as required by the position or school district.
- Knowledge of pediatric physical therapy principles and developmental milestones.
- Understanding of school-based therapy and the role of related services within the IEP process.
- Knowledge of IDEA and California special education requirements as they relate to related services.
- Ability to conduct functional and standardized physical therapy assessments.
- Strong documentation, communication, and organizational skills.
- Ability to manage a school-based caseload and meet required assessment and reporting timelines.
- Ability to collaborate effectively with multidisciplinary educational teams.
- Ability to work independently and exercise sound clinical judgment.
Preferred Qualifications
- Previous experience providing school-based or pediatric physical therapy.
- Experience working with students with developmental disabilities, cerebral palsy, neuromuscular conditions, orthopedic conditions, autism, genetic disorders, and other physical or developmental needs.
- Experience with wheelchair positioning, gait training, adaptive equipment, orthotics, transfers, and mobility supports.
- Familiarity with school-based assessment tools and educationally relevant PT documentation.
- Experience participating in IEP meetings and developing functional IEP goals.
- Experience working with elementary, middle, and/or high-school students.

Physical Requirements
The Physical Therapist must be able to perform the physical requirements associated with school-based therapy, which may include standing, walking, kneeling, bending, reaching, assisting with student positioning and transfers, and handling therapy or mobility equipment, with or without reasonable accommodation.

Pay: $110,000.00 - $145,000.00 per year
