1. ABOUT US
Premier Integrative Health (PIH) is a cash-based functional medicine practice in North Kansas City, built on a simple conviction: health is created, not prescribed. Our founder, Dr. Bradley Dyer, is a board-certified internal medicine physician who completed a fellowship in Integrative Medicine at the University of Arizona under Dr. Andrew Weil and is certified through the Institute for Functional Medicine. He started this practice after years in the conventional system convinced him that suppressing symptoms is not the same as resolving what causes them.
Our physicians, naturopathic doctor, nutritionists, health coaches, nurse, and patient support team work side by side in a collaborative, data-driven practice serving patients across the Kansas City metro and virtually. Patients come to us through our six-month Premier Signature Program and our lifetime Premier Health Collective membership, which means we walk alongside people for years, not appointments.
Our goal is not to make patients dependent on the healthcare system. It is to teach them to become stewards of their own well-being - to eat, move, think, breathe, and live in a way that supports lifelong vitality. 3. ABOUT THIS POSITION
Our Health Coaches help patients turn personalized treatment plans into lasting lifestyle change. Working alongside our physicians, nutritionists, and care team, you will bridge the gap between clinical recommendations and everyday implementation through education, accountability, encouragement, and ongoing support. You’ll manage your own panel of patients across our Premier Signature Program and Premier Health Collective, while leading one weekly group coaching session for our membership community.
In your first 90 days, you’ll build trusted relationships with your patient panel, confidently lead your weekly group session, maintain timely and accurate clinical documentation, and independently manage patient communication and follow-up. As you settle into the role, you’ll also contribute ideas to improve workflows, strengthen the patient experience, and help us continually refine how we deliver care.

4. KEY RESPONSIBILITIESClinical Coaching & Patient Care
- Conduct virtual and in-person health coaching appointments for patients in the six-month Premier Signature Program and the lifetime Premier Health Collective membership.
- Guide patients in implementing individualized care plans using evidence-based lifestyle medicine and functional medicine principles.
- Help patients establish sustainable habits across nutrition, movement, sleep, stress management, mindset, and overall wellness.
- Use motivational interviewing and patient-centered coaching techniques to drive lasting behavior change.
- Identify barriers to success and build realistic action plans with patients — including in complex cases involving trauma, multiple diagnoses, or high reactivity.
- Collaborate directly with physicians and nutritionists on patient progress and care plans.
Patient Journey & Engagement
- Own the accountability relationship for your assigned patients throughout their health journey.
- Monitor patient engagement, spot those falling behind, and intervene proactively with outreach and support.
- Respond promptly to patient portal messages and other patient communications.
- Complete follow-up check-ins between visits to sustain momentum.
- Partner with the Patient Navigator to keep patients on track with appointments, lab testing, program milestones, and care plans.
Documentation & Clinical Operations
- Complete accurate documentation in Cerbo within established timeframes — consistently, without prompting.
- Prepare for upcoming appointments through chart review and visit planning.
- Maintain organized clinical documentation following established workflows and standards.
- Protect dedicated administrative time for charting, messaging, patient follow-up, and care coordination.
- Track patient progress, implementation milestones, and measurable outcomes to help evaluate program effectiveness.
- Surface trends in patient success and opportunities for continuous improvement.
Community Leadership
- Lead one weekly Premier Health Collective (PHC) group session.
- Facilitate group discussions that educate, motivate, and build accountability.
- Participate actively in the PHC WhatsApp community — answering questions, driving engagement, and celebrating patient wins.
- Develop educational resources and community content.
- Attend some evening educational events and community activities.
- As the practice grows, take on additional group programs such as the Premier Metabolic Reset (PMR).
Team Collaboration
- Attend weekly team meetings, case reviews, and ongoing clinical education.
- Coordinate care with physicians, nutritionists, the Patient Navigator, and administrative staff.
- Stay current with evolving research and best practices in functional medicine, lifestyle medicine, and health coaching.
- Contribute ideas that improve patient outcomes, workflows, and the overall patient experience.
- Uphold the PIH mission, culture, and core values, and help sustain a positive, collaborative team environment.
5. SKILLS & QUALIFICATIONS
Required
- Prior health coaching experience in a functional, integrative, or lifestyle medicine clinic.
- Confidence working with complex cases - trauma histories, multiple diagnoses, and high reactivity.
- Consistent time management: notes completed on time, appointments started on time, and follow-ups handled without reminders.
- Excellent communication and motivational interviewing skills - direct, honest, and kind.
- Tech-savvy and quick to learn new systems, including comfort using AI tools. We run on Cerbo, Slack, Asana, Zoom, and Google Workspace.
- A personal health practice that reflects what we teach our patients. Not perfection - alignment.
- A commitment to this role long term. This is not a stepping stone position.
Preferred (Nice to Have)
- One to two years of health coaching experience.
- Certification in functional medicine health coaching or lifestyle medicine - NBHWC, FMCA, IFM/ADAPT, FDN, NTP, or similar.
- Hands-on experience working in Cerbo.
- Located in the Kansas City metro area.
- Experience facilitating group classes or programs.

7. COMPENSATION & BENEFITS
Pay: $30–$35 per hour, based on experience.
Schedule: Part-time at 20–25 hours per week to start, with anticipated growth to full time (30+ hours) as patient volume increases with the growth of the practice. Typical work hours are Monday–Friday, 9am–5pm. Recurring (1-2x/monthly) evening availability is required for Premier Health Collective group sessions, typically 6:00–7:00 pm. Rare weekend availability for one to two practice events per year.
Location: Primarily remote, with the ability to work onsite at our North Kansas City office approximately one day per week preferred.
Reports to: Practice Manager.
Target start: Fall 2026.

8. WHAT SUCCESS LOOKS LIKE HERE
Our coaches do not simply conduct appointments - they become trusted partners in each patient’s health journey. You will know you are thriving here when your patients are implementing the changes their care plan calls for and telling you about it, when your charts and follow-ups are current without anyone asking, and when your weekly PHC session is a room people do not want to miss.
Over time, success means the physicians trust your judgment on your panel, the team counts on your communication, and the workflow around this role runs better because you improved it. Most of all, it means patients who were stuck are moving again - and they credit you with helping them get there.
