Job Title: Clinical Pharmacist

Job Summary: The Clinical Pharmacist is a licensed professional responsible for delivering safe and appropriate pharmacy services to Option Care Health patients. This entry-level role involves coordinating patient services, monitoring patient progress, and educating patients, caregivers, and healthcare professionals on proper medication use. The Clinical Pharmacist plays a critical role in transitioning patients to home care, ensuring they receive high-quality, personalized pharmacy care.
Key Responsibilities:
- Patient Assessment: Evaluate and collect patient-specific information to determine their suitability for home care services, ensuring their medical condition and prescribed therapy align with home care goals.
- Pharmacy Care Plans: Develop, monitor, and evaluate individualized pharmacy care plans for home care patients. Provide recommendations for modifications to therapy or monitoring as needed.
- Patient Education: Educate patients, caregivers, and healthcare staff on medication use, utilizing effective communication and teaching techniques.
- Medication Management: Prepare and dispense medications accurately, adhering to safety and quality assurance practices. Ensure compliance with federal and state laws, including DEA regulations.
- Equipment Management: Maintain and troubleshoot medication administration equipment, providing guidance on its use and resolving any issues.
- Drug Information: Provide timely, evidence-based responses to drug information requests from healthcare providers and patients.
- Compliance: Ensure adherence to accreditation, legal, regulatory, and safety requirements while maintaining patient confidentiality.
Basic Qualifications:
- Education: Graduate of an accredited School of Pharmacy.
- Licensure: Active and unrestricted Pharmacy license granted by the State Board of Pharmacy.
