Job Overview
We are seeking a highly skilled and compassionate Nurse Coordinator - Registered Nurse to join our healthcare team. This role involves overseeing patient care, coordinating clinical activities, and ensuring compliance with healthcare standards across various specialties including acute care, geriatrics, behavioral health, and outpatient services. The ideal candidate will possess extensive nursing experience, proficiency with electronic health record systems such as Epic and Cerner, and a strong understanding of medical documentation, coding, and patient assessment. Responsibilities
- Coordinate and supervise daily clinical operations within the healthcare facility, ensuring adherence to hospital policies and regulatory standards such as HIPAA and NCQA standards.
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations.
- Administer medications, injections, IV infusions, and perform venipuncture with aseptic technique.
- Manage complex procedures such as catheterization (urinary, cardiac), tube feeding, ostomy care, and ventilator management.
- Document all patient interactions accurately using EMR/EHR systems like Athenahealth and eClinicalWorks; ensure compliance with medical records regulations.
- Provide specialized care in areas such as ICU, trauma medicine (Level I & II Trauma Center experience), emergency medicine, hospice & palliative medicine, dialysis, and infectious disease care.
- Lead discharge planning, case management, and coordinate post-acute care including memory care and long-term care settings such as nursing homes or assisted living facilities.
- Collaborate with multidisciplinary teams on diagnostic evaluations including laboratory testing (phlebotomy), medical imaging (sonography), and pathology reports.
- Educate patients and families on health coaching topics like nutrition, medication administration, wound care (including suturing), and disease management strategies.
- Ensure infection control protocols are strictly followed to maintain aseptic techniques during procedures like sterile processing or surgical interventions in outpatient or ambulatory surgery centers.
Skills
- Extensive experience with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
- Proficiency in advanced nursing procedures including IV insertion/infusion, suturing, catheterization (urinary/cardiac), ventilator management, and tube feeding.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding for procedures.
- Ability to perform comprehensive patient assessments across diverse populations including pediatrics (neonatal ventilator), geriatrics (memory/dementia care), behavioral health patients, and those with disabilities or developmental challenges.
- Familiarity with critical care environments such as ICU or trauma centers; experience in emergency room or urgent care settings highly desirable.
- Skilled in infection control practices and maintaining sterile environments during procedures requiring aseptic technique.
- Excellent communication skills for teaching patients about their health conditions and collaborating effectively within multidisciplinary teams.
- Knowledge of Workers' Compensation law applicable to occupational health cases; experience managing complex discharge planning for post-acute or sub-acute care settings is advantageous.
