Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
- Administer medications and injections accurately following HIPAA regulations and clinical confidentiality policies.
- Assist with complex procedures such as IV infusions, catheterizations, wound suturing, and sterile processing techniques.
- Monitor patients’ conditions closely—managing acute pain, ventilator support, airway management, and infection control protocols—especially in critical care or emergency settings like Level I Trauma Centers or ICU environments.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients with diverse needs including Alzheimer’s disease, dementia, developmental disabilities, or post-acute recovery.
- Conduct patient education on medication administration, nutrition (including tube feeding), discharge planning, and health coaching to empower patients for better self-care.
- Document all nursing activities thoroughly in EMR/EHR systems such as Athenahealth or eClinicalWorks to ensure compliance with healthcare standards and facilitate seamless communication across providers.
Requirements
- Valid LPN license with current state licensure; prior experience working in hospital medicine, skilled nursing facilities, or long-term care settings is highly preferred.
- Demonstrated proficiency with EMR/EHR systems like Epic or Cerner; familiarity with ICD-9/10 coding for accurate documentation is a plus.
- Critical care experience including ICU or emergency room work; experience with Level I or Level II trauma centers enhances your candidacy.
- Strong knowledge of anatomy, physiology, pharmacology, and medical terminology to perform comprehensive patient assessments effectively.
- Experience managing complex cases such as hospice care, infectious disease treatment, pain management strategies, or working with geriatric populations.
- Ability to adapt quickly to new environments while maintaining high standards of patient safety and clinical excellence.
- Excellent communication skills for triage assessments and collaborating within multidisciplinary teams dedicated to delivering exceptional patient-centered care.
