We seek a compassionate and skilled Registered Nurse (RN) with outpatient clinic experience in cardiology to join our team. The ideal candidate will support our cardiologists in delivering high-quality patient care, including assessments, coordination of care, patient education, and remote monitoring services for patients with cardiovascular conditions. The RN will work closely with a multidisciplinary team to improve patient outcomes and ensure a positive experience.
Key Responsibilities
- Patient Care and Assessment
- Perform comprehensive assessments, monitor vital signs, and evaluate cardiovascular health.
- Assist in developing and implementing individualized care plans for patients with cardiac conditions.
- Prepare patients for diagnostic procedures, such as EKGs, echocardiograms, stress tests, and CTA coronary imaging, providing pre-and post-procedure care and education.
- Care Coordination and Remote Monitoring
- Collaborate with cardiologists and healthcare team members to coordinate patient care.
- Help manage our cardiology remote monitoring services, ensuring timely review and follow-up on patients’ cardiovascular data.
- Schedule and follow up on diagnostic tests, procedures, and appointments as needed.
- Communicate effectively with other medical professionals to ensure continuity of care.
- Patient Education and Counseling
- Educate patients and families on cardiovascular conditions, medications, lifestyle modifications, and self-care techniques.
- Provide guidance to patients managing chronic conditions, such as heart failure, hypertension, and arrhythmias.
- Document patient interactions, education, and follow-up actions accurately in the electronic medical record (EMR).
- Clinical Support and Procedures
- Administer medications, monitor for adverse reactions, and educate patients on medication compliance.
- Perform minor in-office procedures, such as injections, wound care, and peripheral IV line insertions, as permitted by the state’s scope of practice.
- Respond to patient phone calls and inquiries, triage appropriately, and provide guidance on symptom management.
- Quality Improvement
- Participate in quality improvement initiatives and adhere to clinic policies to maintain high standards of patient care.
- Assist in developing clinic protocols and patient care workflows to enhance service delivery and patient satisfaction.
