Physical Therapist Assistant (PTA) – Home Health
Ideal Choice Home Health is seeking a compassionate and dependable Physical Therapist Assistant (PTA) to join our growing home health team.
This is an opportunity to provide meaningful one-on-one care to patients in the comfort of their own homes while working alongside an experienced clinical team.
Position: Physical Therapist Assistant (PTA)
Employment Type: Contract / Part-Time / Per Diem
Setting: Home Health
Location: San Fernando Valley
Pay: Competitive per-visit compensation based on experience and qualifications
What You'll Do
As a PTA with Ideal Choice Home Health, you will work under the direction of a licensed Physical Therapist to provide quality rehabilitation services to homebound patients.
Responsibilities include:
- Provide physical therapy treatments and interventions according to the patient's individualized plan of care
- Assist patients with therapeutic exercises, mobility, strengthening, balance, and functional activities
- Monitor and document patient progress and response to treatment
- Educate patients and caregivers on exercises, mobility, safety, and home programs
- Communicate changes in patient condition to the supervising Physical Therapist and appropriate members of the care team
- Complete timely and accurate clinical documentation
- Help patients improve their independence and ability to safely function in their home environment
- Maintain professional relationships with patients, families, physicians, and other members of the interdisciplinary team
- Follow all applicable Medicare, state, and agency requirements
What We're Looking For
- Current California Physical Therapist Assistant license
- Graduate of an accredited Physical Therapist Assistant program
- Current CPR certification
- Previous home health experience is preferred but not required
- Strong clinical skills and attention to detail
- Excellent communication and interpersonal skills
- Reliable transportation and a valid driver's license
- Ability to work independently and manage a daily patient schedule
- Strong organizational and documentation skills
- A compassionate, patient-centered approach to care
