Overview:
Working hours are Tuesday, Thursday and Saturday 8:30- 5 or 8-4:30pm if available. Training MUST be Monday - Friday for the first few weeks
Responsibilities: As a member of the multidisciplinary team, the Care Management Assistant provides discharge planning support to the Care Manager and Social Worker and supports patients and families / caregivers by reducing non-clinical barriers to discharge in the Inpatient and ED environments. Qualifications:
Education-
H.S. Diploma or General Education Degree (GED) Required
- Associate’s Degree or two years of college in human services/health related discipline Preferred
Work Experience-
2 years of experience in a healthcare or social services setting Required
