Foundational Community Supports Case Manager

Overview
We are seeking a dedicated, mature, and self-motivated Foundational Community Supports Case Manager to provide supportive housing and supported employment services to clients who may face significant barriers to housing stability, employment, health, transportation, and community connection. This role is ideal for someone who is compassionate, organized, ethical, and comfortable working independently while meeting clients where they are in the community.
The Case Manager will support clients in identifying goals, building skills, accessing resources, attending appointments, preparing for employment opportunities, and maintaining progress toward stable housing and meaningful work. This position requires strong documentation habits, sound boundaries, excellent time management, and the ability to engage respectfully with diverse populations who may need consistent encouragement, motivational interviewing, guidance, and practical support.
About Foundational Community Supports in Washington State
Foundational Community Supports, often referred to as FCS, is a Washington State Medicaid benefit that provides supportive housing and supported employment services to eligible Apple Health clients with complex needs and significant barriers to stability. FCS is designed to help participants obtain and maintain safe, stable housing and meaningful employment by providing individualized coaching, advocacy, resource connection, skill-building, service coordination, and ongoing support.
The program focuses on helping clients build the skills, confidence, and support systems needed to move toward greater independence, recovery, self-sufficiency, and long-term stability.
Key Responsibilities
· Provide person-centered case management services for clients receiving supportive housing and/or supported employment services.
· Conduct assessments to identify client needs, strengths, barriers, goals, and readiness for housing or employment support.
· Develop and implement individualized service plans in collaboration with clients and other appropriate stakeholders.
· Assist clients with housing searches, landlord communication, lease-readiness skills, appointment preparation, and housing stability planning.
· Support clients with employment exploration, job readiness, applications, interviews, workplace communication, and employment retention.
· Use motivational interviewing, coaching, and strengths-based engagement to help clients build confidence and follow through on goals.
· Provide transportation to and from appointments, interviews, court dates, community resources, and other important events as needed.
· Spend a significant portion of work time in the community, meeting clients where they are and supporting them in real-world settings.
· Coordinate referrals and services with community partners, healthcare providers, behavioral health providers, housing resources, employers, courts, and other systems of support.
· Maintain appropriate professional boundaries while providing compassionate, consistent, and practical support to clients who may require a high level of guidance.
· Manage an active caseload of approximately 20-30 clients, depending on program needs and service intensity.
· Participate in team meetings, staffing discussions, trainings, and program development activities as required.
Documentation, Billing, and Compliance Expectations
· Complete accurate, timely, ethical, and Medicaid-compliant case notes for all billable services.
· Document client contacts, service activities, progress, barriers, outcomes, and follow-up needs clearly and professionally.
· Maintain strong organization around appointments, authorizations, deadlines, caseload activity, documentation requirements, and service plans.
· Use SetWorks for documentation, service tracking, billing support, and case management tasks; prior SetWorks experience is preferred but not required for a strong learner.
· Demonstrate integrity, confidentiality, professional judgment, and ethical practice in all client interactions and records.
Ideal Candidate
· Has experience in case management, supportive housing, supported employment, behavioral health, human services, peer support, recovery support, workforce development, or a related field.
· Communicates clearly, respectfully, and professionally with clients, coworkers, providers, employers, landlords, and community partners.
· Is comfortable working with diverse populations, including individuals experiencing housing instability, behavioral health challenges, substance use recovery, justice involvement, trauma, disability, poverty, or other complex barriers.
· Can balance compassion with boundaries and provide support without creating dependency.
· Works well with limited oversight and can manage time, priorities, appointments, documentation, and follow-up independently.
· Has strong computer skills and the ability to learn new systems quickly.
· Demonstrates maturity, reliability, sound judgment, patience, and professionalism in challenging situations.
· Is comfortable working in the community and adapting to changing client needs, schedules, locations, and service priorities.
· Understands the importance of ethical documentation, confidentiality, client dignity, and trauma-informed service delivery.
Qualifications
· Experience in case management or a related social services role preferred.
· Strong written documentation skills and ability to complete timely case notes.
· Excellent communication, interpersonal, problem-solving, and decision-making skills.
· Ability to work effectively with individuals from diverse backgrounds and life experiences.
· Knowledge of community resources, housing systems, employment supports, behavioral health resources, or social service systems preferred.
· Peer counseling, recovery coaching, chemical dependency, social work, workforce development, or related credentials are preferred but not required.
· Lived experience is valued and preferred when paired with strong professional boundaries and ethical practice.
· Valid driver’s license, reliable transportation, and ability to transport clients as needed.
· Ability to use a computer proficiently, including email, online documentation systems, scheduling tools, and Microsoft Office or similar programs.
Schedule and Work Environment
This is a full-time, 40-hour-per-week position with a flexible schedule based on client needs, appointments, documentation requirements, and program expectations. Approximately 75-80% of the role is community-based, which may include meeting clients in housing settings, community locations, employment sites, appointments, interviews, courts, resource centers, and other service-related locations.
