Job Responsibilities of Direct Support Professional (DSP)

Provides transportation for individual(s), by picking up individuals from their homes and transporting them to the day hab site; and transporting the individual(s)’ home from the day hab site, using the company’s vehicle.

Transports individual(s) for community integration/inclusion activities using the company’s vehicle.

Creates and maintains an environment that is healthy, safe, comfortable and productive for the individual(s) serviced.

Travels out into the community with the individuals to participate with the individual(s)’ with their community integration and inclusion activities to fulfill their individual(s) goals

Follows all agency’s policies and procedures regarding safety in all environments.

Implements individual(s)’ support plans as written.

Provides assistance and support to individual(s) in activities of daily living.

Obtains knowledge of individual(s) supported desired outcomes and goals; works to ensure they are integrated into daily activities.

Maintains strict confidentiality of all personal information concerning individual(s) supported.

Treats all individual(s) supported, families, coworkers and supervisors with dignity and respect.

Demonstrates the importance of community involvement, social interactions, and maintaining relationships.

Encourages individual(s) supported to give back to the community.
-
Exhibits professional behavior towards the individual(s) supported, families, coworkers, and supervisors.

Provides clear, accurate and timely records and documentation.

Participates in and completes periodic training and certification as required.
-
Reports all concerns to the appropriate supervisor in a timely manner.

Reports all instances of abuse, neglect, mistreatment and exploitation immediately to the appropriate supervisor.

Completes Individuals’ Monthly notes demonstrating the progress of the individuals on a timely basis

Perform all other tasks required or assigned by supervisor.
