Clinical Coordination: Coordinates clinically with the Director of Nursing or designated Clinical Supervisor.
Scope Summary: Provides skilled nursing services, patient assessments, OASIS documentation, care coordination, and clinical oversight in accordance with physician orders and applicable home health regulations.
Qualifications
· Active Florida Registered Nurse license in good standing.
· Minimum one year of nursing experience; home health experience preferred.
· Current CPR certification.
· Valid driver license and automobile insurance.
· Ability to maintain accurate clinical documentation.
Responsibilities
· Perform skilled nursing visits and assessments.
· Complete admission, transfer, recertification, resumption of care, and discharge documentation.
· Coordinate care with physicians, therapists, caregivers, and agency clinical personnel.
· Monitor patient condition and communicate significant changes appropriately.
· Provide patient and caregiver education.
· Maintain timely and accurate clinical records.
· Provide onsite supervision of assigned LPNs and HHAs when applicable.
· Perform services in accordance with Florida regulations and professional nursing standards.
Scheduling & Assignments
May accept or decline assignments based on availability and professional judgment. Compensation may be structured on a per-visit, per-shift, or per-service basis.
Compliance Requirements
Shall maintain all required licenses, certifications, screenings, trainings, and insurance necessary to perform contracted services in compliance with Florida home health regulations.

Agree to maintain confidentiality of all patient information and comply with HIPAA and all applicable privacy laws.
Pay: $30.00 - $35.00 per hour
