Responsibilities:
Identify each resident’s social, emotional and psychological needs.
Develop and carry out a plan to develop the residents’ full potential during their stay.
Planning for the resident’s discharge or transition to another care environment.
Develop social assessment and care plan, which identifies medically related social and emotional problems and needs with realistic goals and specific actions to be taken
Help coordinate pre-admission and admissions processes
Ability to listen and act compassionately with residents/family members; must be able to work on own and maintain job related jobs
Document residents’ behavioral and psycho-social status
Provide individual and group help for family and residents at all times of adjustment, crisis.
Orients resident and family to Nursing center, helping both adjust to new environment , ensuring they feel welcome.
Requirements:
Must possess, as a minimum, a high school diploma or its equivalent.
Completion Certificate for approved Social Services training program (preferred).
One (1) or more years or work in a skilled nursing facility, hospice or long-term acute care (preferred).
Must be able to pass a background check.
