Position Summary:
Performance Home Health is seeking an experienced and compassionate Physical Therapist Assistant (PTA) to join our growing home health team.
In this field-based role, the PTA provides individualized physical therapy services in patients' homes under the direction and supervision of a licensed Physical Therapist. The ideal candidate is organized, clinically confident, patient-focused, and passionate about helping individuals improve their mobility, strength, balance, safety, functional independence, and overall quality of life.
Key Responsibilities:
- Provide skilled physical therapy treatments in accordance with the established Plan of Care and under the supervision of a licensed Physical Therapist.
- Implement individualized interventions focused on strength, mobility, balance, endurance, fall prevention, safety, and functional independence.
- Monitor patient progress and communicate changes in condition, outcomes, or clinical concerns to the supervising Physical Therapist.
- Educate patients and caregivers regarding therapeutic exercises, home programs, mobility techniques, fall prevention, and safety.
- Complete accurate and timely clinical documentation within the electronic medical record (EMR).
- Maintain compliance with agency policies, regulatory requirements, and professional standards of practice.
- Collaborate with Physical Therapists and interdisciplinary care team members to support coordinated, patient-centered care.
- Follow infection-control protocols and established patient-safety procedures.
- Travel throughout assigned service areas to provide therapy services in patients' homes.
Required Qualifications:
- Graduate of an accredited Physical Therapist Assistant program
- Current and unrestricted Physical Therapist Assistant license in the state of practice
- Current CPR certification
- Valid driver's license, automobile insurance, and reliable transportation
- Strong clinical judgment, communication, organizational, and time-management skills
- Ability to work independently and effectively manage a field-based patient schedule
Preferred Qualifications:
- 3+ years of home health experience preferred
- Experience utilizing electronic medical record (EMR) systems
- Demonstrated ability to independently manage a flexible patient schedule
- Strong knowledge of therapeutic exercise, functional mobility, fall prevention, and patient safety within the home health setting
