Responsibilities
- Deliver attentive patient observation to monitor health status and identify any changes requiring intervention
- Assist with activities of daily living such as bathing, dressing, grooming, toileting, and mobility support
- Administer medications accurately following home healthcare regulations and medical documentation protocols
- Perform vital signs assessments including blood pressure, pulse, respiration, and temperature readings
- Support clients with specialized care needs such as Alzheimer’s care, dementia care, hospice support, and end-of-life assistance
- Operate Electronic Health Record (EHR) systems to document patient information securely and efficiently
- Utilize equipment like Hoyer lifts and perform catheter care while maintaining strict adherence to HIPAA privacy standards
- Prepare nutritious meals and assist with light housekeeping tasks including cleaning and laundry to promote a safe living environment
- Provide compassionate companionship and emotional support to enhance overall quality of life for clients
- Follow safety procedures including first aid and basic life support (CPR), especially during emergencies or incidents
Skills
- Strong patient observation skills with the ability to recognize subtle health changes
- Prior assisted living or long-term care experience preferred; experience working with individuals with developmental disabilities is a plus
- Knowledge of medical terminology, medication administration, and home & community care regulations
- Proficiency in EMR/EHR systems for accurate medical documentation and record keeping
- Experience caring for geriatric patients, including dementia and Alzheimer’s disease management
- Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
- Excellent caregiving skills combined with compassion, patience, and effective communication abilities
- Familiarity with hospice care practices and end-of-life support techniques
- Capable of working independently in home health settings while maintaining high standards of patient confidentiality under HIPAA guidelines
- Wellness Care Concierge is a Florida-registered Nurse Registry. All applicants are required to successfully complete a Level 2 Background Screening through the Florida Care Provider Background Screening Clearinghouse as a condition of employment or placement.
