Job Summary
Hildora Home Health is seeking an experienced RN Clinical Manager & Community Liaison to help launch and grow our skilled home health agency. This is a key leadership role for a nurse who understands quality patient care, home health operations, compliance, and professional referral relationships.
The ideal candidate is a licensed RN with home health experience who can supervise clinical services, support documentation standards, coordinate patient care, and represent Hildora Home Health with hospitals, physicians, rehab centers, skilled nursing facilities, assisted living communities, and other referral partners.
Responsibilities
- Provide clinical leadership for skilled home health services.
- Supervise, coach, and support nursing and field staff.
- Review plans of care, physician orders, documentation, and clinical records.
- Help maintain compliance with agency policies, Medicare home health expectations, and survey readiness.
- Coordinate patient admissions, starts of care, recertifications, and care-plan updates.
- Communicate professionally with patients, families, physicians, referral sources, and internal staff.
- Support quality improvement through chart review, documentation follow-up, and care coordination.
- Build ethical referral relationships with hospitals, physician offices, rehab centers, SNFs, assisted living facilities, and community partners.
- Educate referral sources and community partners about Hildora Home Health services.
- Track outreach activity, referral trends, and growth opportunities.
- Assist leadership with hiring, onboarding, training, and agency launch priorities.
Required Qualifications
- Active RN license in good standing.
- Home health, skilled nursing, hospice, case management, or closely related clinical experience.
- Strong knowledge of care plans, physician orders, clinical documentation, and patient coordination.
- Ability to supervise staff and communicate clearly with clinical and non-clinical partners.
- Valid driver's license, reliable transportation, and ability to travel locally.
- Strong organization, follow-through, and professional judgment.
Preferred Qualifications
- Prior experience as a Home Health Clinical Manager, DON, ADON, RN Case Manager, Branch Director, or similar leadership role.
- Familiarity with Medicare-certified skilled home health operations.
- Experience with EMR systems and chart audits.
- Existing referral relationships in the local healthcare community.
- Healthcare marketing, liaison, intake, or business development experience.
- Experience helping launch or grow a healthcare agency.
Pay: $70,000.00 - $100,000.00 per year
