We are seeking a highly organized and patient-focused medical coordinator to support HNYMPC's population health and quality initiatives through proactive patient outreach and scheduling. This position is responsible for identifying and contacting patients who are due for preventive screenings, and other quality gap closure services. The position works collaboratively with providers, clinical staff, and patients to improve preventive care compliance, increase appointment completion rates, and support organizational quality goals.
The ideal candidate possesses excellent communication and customer service skills, enjoys engaging with patients by telephone, and thrives in a fast-paced healthcare environment.
Responsibilities include:
- Conduct outbound and inbound calls to schedule patients for preventive screenings, and other quality gap closure appointments.
- Review daily reports and work queues to identify patients due for preventive services and quality measures.
- Schedule appointments for services including, but not limited to:
- Mammograms
- Colorectal Cancer Screening
- Cervical Cancer Screening
- Diabetic Eye Exams
- Kidney Health Evaluations
- Blood Pressure Follow-up
- Diabetes laboratory testing (A1C, Microalbumin, etc.)
- Immunizations
- Other preventive care services as assigned
- Annual wellness visits
- Educate patients on the importance of preventive care and encourage completion of recommended screenings.
- Verify and update patient demographic, insurance, and contact information.
- Confirm appointments and provide reminder calls or messages to improve appointment adherence.
- Reschedule missed or canceled appointments to ensure timely completion of quality measures.
- Coordinate with provider offices and internal departments to ensure appointment availability and timely scheduling.
- Document all patient outreach attempts, scheduling activities, and patient interactions accurately in the electronic medical record (EMR).
- Assist patients with basic scheduling questions and direct clinical inquiries to the appropriate healthcare staff.
- Maintain daily productivity and quality standards for patient outreach and appointment scheduling.
- Protect patient confidentiality and comply with HIPAA and organizational policies.
- Participate in department meetings and quality improvement initiatives.
- Perform other duties as assigned.
Qualifications/Education
- High School Diploma or GED required.
- Degree in Healthcare Administration, Business, or a related field preferred.
Experience
- Minimum of two (2) years of experience in healthcare scheduling, patient outreach, call center, medical office, or customer service preferred.
- Experience working with electronic medical records (EMR/EHR) preferred.
- Experience supporting population health, preventive care, HEDIS, or quality initiatives is a plus.

- Knowledge of preventive healthcare and quality measures is preferred.
- Commitment to maintaining patient confidentiality and HIPAA compliance.
Pay: $50,000.00 - $75,000.00 per year
