We are seeking an experienced Nurse Practitioner to join our team in a Skilled Nursing and Rehabilitation team. The ideal candidate is a compassionate, patient-focused provider with experience caring for geriatric and post-acute patients.
Responsibilities:
- Provide comprehensive medical care to residents in a skilled nursing and rehabilitation setting.
- Perform patient assessments, diagnose and treat acute and chronic conditions.
- Develop and manage individualized treatment plans.
- Collaborate with physicians, nursing staff, therapists, and interdisciplinary teams.
- Document patient care accurately and maintain compliance with regulatory standards.
- Respond to changes in patient conditions and provide timely follow-up care.
Qualifications:
- Active Connecticut Nurse Practitioner (APRN) license required.
- Current DEA registration .
- Minimum of 1 year of experience as a Nurse Practitioner in a hospital, outpatient skilled nursing, rehabilitation, long-term care, or post-acute care setting.
- Strong clinical assessment and decision-making skills.
- Excellent communication and teamwork abilities.
- Will cover malpractice insurance
Preferred Qualifications:
- Experience with geriatric or post-acute patient populations.
- Familiarity with electronic medical records (EMR) systems.
Pay: From $75.00 per hour
