Location: San Diego North Inland
Key Responsibilities
- Provide physical therapy treatments as ordered by the attending physician, dentist, or podiatrist.
- Conduct comprehensive patient evaluations and assist in developing and updating the Plan of Care (POC).
- Assess functional levels and collaborate with the physician regarding patient progress.
- Observe, document, and report changes in patient condition to the physician, case manager, and interdisciplinary team.
- Maintain timely, accurate, and compliant clinical documentation in accordance with Lorian Health policies and regulatory requirements.
- Educate and instruct patients, families, and caregivers on therapy programs and safety measures.
- Supervise and guide healthcare personnel as appropriate.
- Evaluate the home environment and recommend modifications to enhance safety and independence.
Qualifications
- Current, active license to practice as a Physical Therapist in the state of employment.
- Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
- Current CPR certification.
- Valid driver’s license and insured vehicle meeting Lorian Health requirements.
- Pre-employment health clearance, including negative TB test/CXR and other state-required screenings.
- Ability to accurately document patient care and follow written care plans.
- Self-motivated with the ability to work independently with minimal supervision.
- Maintains good physical stamina and overall mental health necessary to perform job duties.

This position is based on a Pay per visit compensation model.
Pay per visit ranges from $105-$150 visit commensurate with experience and depending on type of visit. The compensation reflected on this posting is an estimate of yearly compensation.
