We’re on the lookout for a detail-driven MDS Coordinator (RN or LPN) to join our amazing team at Scenic Wellness & Rehabilitation! If you’re passionate about accuracy, teamwork, and quality resident care, this is your place to shine.
Oversee and complete the MDS and care management process from admission to discharge
Work closely with the Interdisciplinary Team to ensure compliance with state and federal guidelines
Maintain accurate documentation and data entry in MDS software
Participate in meetings to review resident care, assessments, and quality measures
Train and support nursing staff on MDS processes and PPS/Medicaid requirements
Ensure care plans reflect residents’ current needs and changes
RN or LPN license in Missouri (required)
2+ years of experience in a skilled nursing facility (preferred)
Strong knowledge of MDS 3.0, Medicare PPS, Medicaid Case Mix, and OBRA regs
Attention to detail and a heart for resident care
