The Critical Time Intervention (CTI) Program is an evidence - based, phase-based, time limited program designed to assist patients in accessing needed services during critical transition periods.
Purpose of Position
A Care Manager at the Interborough Developmental & Consultation Center (IDCC) in Brooklyn, NY, plays a crucial role in supporting individuals with mental health needs. They act as a resource to coordinate various aspects of an individual’s life, including housing, employment, social relationships, and access to community resources, all within a holistic and person-centered approach. This role involves not only coordinating traditional mental health services but also addressing the individual’s overall well-being.
The CTI Program is an evidence-based, time limited program designed to assist patients in accessing needed services during critical transition periods.
Responsibilities
- Carry a caseload of 15.
- Develop person-centered care plans.
- Share knowledge of community resources
- Provide linkage to resources
- Engage individuals in skill building
- Offer crisis management
- Collaborate with other providers
- Assist individuals in navigating complex systems
- Work with informal support.
- To help individuals receive the level of and/or have access to the services/supports they choose.
- To monitor and track progress of individuals in assigned caseload; participate in the development of individualized rehabilitation plans.
- Travel or Special Conditions/Requirements: Travel Required
- Assisting individuals improve self-management of psychiatric symptoms and reduce episodes of substance use relapse, impacting a person’s functioning in home, school, work, and community.
- Rehabilitation support necessary for the individual to implement learned skills so the person can remain in a preferred natural community location that may include: home or residences.
- Assisting with identifying meaningful life goals and objectives, coping skills training, stress and anger management, and improving personal autonomy,
- Assisting with development and pursuit of leisure and recreational interests.
- Assisting with transportation navigation
- Assisting with accessing health-care professionals, managing chronic medical conditions, and medications and establishing good health routines and practices.
- Provision of psycho-education, social skills development, dietary and wellness education.
- Training/providing assistance with attention to daily living skills
- Training/providing assistance with benefits and financial management.
- Employment- including assistance in pursuing, securing and retaining paid or unpaid employment and support in sustaining employment through ongoing counseling, mentoring and advocacy; support to remain in school and academic achievement.
- Attend required management and staff meetings. Complete CPI and organizational training.
- Other projects as assigned.
Requirements
- High school diploma, GED, or Bachelor's degree in psychology, human services or related field and at least six months of direct care experience with individuals with serious mental illness (SMI)
- Experience in behavioral health, substance use and/or other social services settings.
- Knowledge of computers and office applications for electronic medical records documentation.
- Willingness to work collaboratively with others.
- Well organized with strong time management skills.
- Enthusiasm and self-motivation regarding identifying client needs and obtaining related resources.
- Able to work independently and as a member of a team.
- Excellent interpersonal skills, including being able to maintain a professional temperament with both clients and colleagues.
- Knowledgeable with regard to the availability of community facilities and resources for referral, transfer and discharge planning.
- Strong crisis intervention skills.
- Able to meet program standards in documenting in client records.
- Comfortable and competency working with racially, culturally, religiously and ethnically, linguistically, differently abled diverse communities, including but not limited to BIPOC and LGBTQ+ communities.
