Cottage Home Care Michigan is seeking a compassionate and reliable Per Diem Registered Nurse (RN) to join our growing home care team. In this role, you will deliver high-quality, patient-centered care in the comfort of patients’ homes, ensuring their safety, dignity, and overall well-being. Pay Rate
- Start of Care / New Admission Visit: $200 per visit
- Revisit / Follow-up Visit: $150 per visit
Responsibilities
- Perform comprehensive patient assessments, including vital signs, health history, and ongoing condition monitoring.
- Develop and implement individualized care plans in collaboration with physicians and care teams.
- Administer medications, injections, and treatments as prescribed, following proper clinical protocols.
- Provide skilled nursing care such as wound care, IV therapy, catheter care, and patient education.
- Accurately document all patient interactions and care provided using electronic health record (EHR) systems.
- Monitor patient progress and promptly report any changes in condition to the appropriate healthcare providers.
- Educate patients and families on disease management, medications, and home care techniques.
- Ensure compliance with all state regulations, agency policies, and HIPAA standards.
Requirements
- Active and valid Registered Nurse (RN) license in the state of Michigan.
- Minimum of 1 year of nursing experience; home care experience preferred.
- Strong clinical assessment and critical thinking skills.
- Proficiency in EHR/EMR systems for accurate documentation.
- Ability to work independently and manage time effectively in a home care setting.
- Excellent communication and interpersonal skills.
- Current CPR certification (preferred).
- Reliable transportation and willingness to travel to patient homes.
