Job Overview
Our Client is seeking a compassionate and dedicated Certified Home Health Aide to provide high-quality in-home care to clients with diverse needs. The ideal candidate will have experience in patient observation, assisting with Activities of Daily Living (ADLs), and working within home health or long-term care settings. Responsibilities
- Assist clients with ADLs such as bathing, dressing, grooming, and mobility support.
- Monitor and document patient condition, vital signs, and medical history using EMR/EHR systems.
- Administer medications and assist with medication management following prescribed instructions
- Support patients with memory care needs, including Alzheimer's and dementia care.
- Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely.
- Provide companionship and emotional support to clients, fostering a caring environment.
- Assist with meal preparation and light housekeeping tasks including laundry and cleaning.
- Maintain accurate medical records and ensure compliance with HIPAA regulations.
- Collaborate with healthcare teams to coordinate patient care plans and report any changes in condition
- Follow safety protocols for hospice, hospice care, or patients with disabilities
Requirements
- Valid certification as a Home Health Aide (HHA) in good standing.
- Experience working with individuals with developmental disabilities, Alzheimer’s/dementia, or memory care needs.
- 6 months medical & physical completed by a physician.
- 6 months drug test screening completed.
- PPD test screening with proof.
- Rubella & measles vaccination proof.
- Strong communication & computer skills.
- Ability to follow directions, keep records, and travel case to case.
- Reliable transportation.
- Ability to care for patients in a home-based setting.
- Ability to operate EMR/EHR systems accurately and efficiently.
- Strong patient observation skills and the ability to work compassionately with diverse populations.
- Excellent communication skills and ability to work independently or as part of a team.
- Reliable transportation and valid driver’s license preferred for home visits.
