JOB SUMMARY
The Public Health Services Division is a part of the integrated Haywood County Health and Human Services Agency, which includes the Haywood County Health Department and the Haywood County Social Services Department. The purpose of the Public Health Services Division is to provide preventative health care to the citizens of Haywood County. Services provided by this are primary, secondary and tertiary.
The primary purpose of this position is to provide support to individuals with substance use disorders and their families. Provide community members with resources for social needs, mental/behavioral health, and physical health care. Provide evidence-based prevention education to local school-aged children with a focus on middle school students. Responsibility includes offering on-going support and assistance to this community involvement/mobilization effort. This may involve helping lead, facilitate, and coordinate activities and projects related to substance use.
Work Schedule:
Monday through Friday from 8:00 a.m. to 5:00 p.m. with 1 hour for lunch, however an alternate work schedule (40 hours per week) may be considered upon request . Other hours may be set or required per the County policy. Perform duties in times of emergency, including but not limited to, those that may be related to a Public Health Emergency or mass sheltering, as the agency is a mandated shelter for the county, and performs such duties as assigned, and the hours worked in emergency response could vary from hours worked in the office.
ESSENTIAL JOB FUNCTIONS:
a. Work with justice-involved persons and persons with substance use issues to identify resources for housing, healthcare, food insecurities, employment, substance use treatment (if desired), transportation, and other social services they may need as they are available.
b. Collaborate with area agencies and community partners on overdose prevention strategies.
c. Build and maintain referral networks with local and regional partners including, but not limited to: sober loving facilities, rehabilitation facilities, community colleges, substance use support groups, primary care providers, NCWORKS representative, tobacco cessation resources, and housing assistance and authority programs.
d. Provide evidence-based prevention education to local school-aged children with a focus on middle school students. Projects should be focused on substance use prevention and mental health needs.
e. Provides engagement to clients/patients with substance use and/or mental health disorders.
f. Work with patients/clients before, during, and after admission to support continuity of services by building trust and rapport with patients/clients, offering mentorship, emotional support, and encouragement throughout the recovery journey.
g. Provide advocacy and support services from a non-clinical perspective in the outpatient setting.
h. Assists clients/patients in identifying their strengths, developing and practicing healthy coping strategies, and navigating the challenges of recovery, empowering clients/patients to articulate their own goals, reinforce self-efficacy, and promote empowerment through peer connection.
i. Develop relationships with public and private community resources and outreach support to connect clients/patients and provide information/contacts to services, such as housing, food, and medical assistance.
j. Position transports and accompanies clients/patients to facilitate access to services.
k. Educates the public and private sectors concerning importance of community health priorities and the role of HHSA in developing strategies to promote and achieve goals.
l. Represents the program on various committees and boards of related organizations/agencies in the community and assists in their efforts that support the goals of the program by interpreting coalition services, offering collaboration on community activities and sharing expertise on community mobilization and outreach.
m. Monitors, records, and submits monthly reports and statistical data to supervisor regarding community involvement, educational activities, media contacts, and activities of the program.
n. Collects and maintains evaluation information on health education projects. This information is presented to community groups to promote healthy lifestyles and generate on-going support for projects/activities.

- Evaluate patient's needs. Explain clinic procedures, Medicaid application and coverage, refer client to Social Services, inform client of appropriate material to take with them when application is made. Explain services to client if out of County provider is desired. Offer WIC services when applicable.
- Supply patient with lab information sheet and Public Health Services Division brochure
- Receive referral forms from area medical providers to contact patients for appointments and follow up for eligible Public Health Services.
- Instructions are given to patient for filling prescription out of Health Department stock if appropriate.
s. Compile medical records, schedule appointments for Advanced Practice Provider.
t. Fax copies of patient's medical records to other agencies while maintaining HIPAA compliance.
- Employees are required to maintain compliance with all State and Federal laws and program requirements including but not limited to confidentiality laws (HIPAA, Juvenile Code, PII, etc.). Employees are expected to adhere to Agency, Countywide and Departmental policies and procedures regarding information security and confidentiality, and to report any suspected unauthorized disclosure or loss of information immediately. Employees must ensure security of all County data, equipment, and information in both electronic and paper form that is accessed as part of job duties to minimize risk of improper disclosure or data breach.

MINIMUM REQUIREMENTS TO PERFORM WORK:
- Graduation from high school and demonstrated possession of knowledge, skills, and abilities gained through at least one year of office assistant/secretarial experience. Previous Peer Support experience preferred but not required.
- CPR Certification/Recertification every two years
- Must participate in Respiratory Protection Plan and be Fit Tested Annually.
- Must be at least twenty-one years of age to drive a County vehicle.
Valid state driver’s license. Must have no more than three (3) points showing on current DMV records in order to operate County vehicles.

Additional Training/Experience :
- Infection Control Regulatory Practices /OSHA Infection Training required by state
ICS training based on statutory requirements and duties of the job.

- Ability to support adults in both group and one-on-one settings.
- Ability to effectively communicate with community/outreach service providers, persons with substance use and/or mental health disorders, treatment team members, and clinical staff in verbal and written forms.
Ability to develop effective working relationships with clinical professionals involved in treating substance use and/or mental health disorders.

PHYSICAL DEMANDS:
Requires sedentary work that involves sitting or standing virtually all of the time and may involve a moderate to heavy amount of lifting and carrying items from location to location. Considerable skill, adeptness and speed in the use of fingers, hands or limbs in tasks involving repetitive motions and the use of standard office equipment.
The job requires normal visual acuity and field of vision, hearing and speaking abilities, depth and texture perception, and color perception. Must possess ability to ambulate without assistance.
WORK ENVIRONMENT:
The office of this position is located at the Haywood County Health and Human Services Agency. The working conditions of this position are that of a typical office environment. This position requires a significant amount of travel throughout the county and within the region.
