Hospice RN Case Manager ($1500 Sign on Bonus)

Providence is currently seeking a highly motivated RN who is interested in using his/her clinical skills in a Hospice care setting as a daytime Admit Nurse.
Responsibilities and desired skills include:
· Experience as RN Case Manager who understands the profile of patients who qualify for Hospice
· Documents and completes comprehensive assessment and patient care according to CMS/Medicare regulations
· Collaborates well with patients, patient’s family, additional health care providers, and office staff
· Attends and participates in interdisciplinary group meetings
· Possesses strong assessment skills
· Knowledge and management of disease processes and end of life care
· Experience with electronic documentation and basic computer skills.
· Must be detail oriented with demonstrated ability to work independently and organize a workload while managing multiple priorities
· Must have excellent verbal and written English communication skills
· Ability to be flexible, problem-solve, facilitate decision-making, and educate others to the palliative nature of hospice care in a variety of situations.
· Reliable independent means of transportation with valid driver's license and required auto liability insurance.
Job Type: Full-time
Pay: $75,000.00 - $85,000.00 per year
