We're looking for an experienced Hospice Registered Nurse in the greater New Orleans area who is passionate about high-quality, patient-centered care and ready to make a difference in the lives of those we serve.
What You'll Do
- Deliver hands-on, compassionate care to hospice patients in their homes or facilities
- Assess, plan, and update individualized patient care plans in collaboration with the interdisciplinary team (IDT)
- Provide emotional support to patients and their loved ones with empathy and understanding
- Administer medications and treatments as prescribed in the patient’s plan of care
- Communicate regularly with physicians and IDT members about patient progress and needs
- Complete new admissions and follow-up visits as needed
- Support and guide family members and caregivers through the hospice journey
- Recognize and implement best practices for pain and symptom management
- Supervise LPNs during on-call shifts
- Ensure all care complies with state and federal hospice regulations
What we're looking for:
- Two (2+) years of nursing experience in hospice
- Registered Nursing license in the state of Louisiana
- Current CPR certification
