Registered Nurse (RN) Assessment Nurse is responsible for conducting nursing assessments for non-medical in-home care clients to ensure services are appropriate, safe, and consistent with South Carolina Medicaid, DHEC, and agency requirements. The RN completes initial client assessments, reassessments every 30, 60, and every 90 days thereafter, as well as assessments following significant changes in the client’s condition. The RN develops and updates the Plan of Care, provides clinical oversight, and collaborates with clients, families, caregivers, physicians, case managers, and agency staff to promote quality care.
Essential Duties and Responsibilities
Conduct comprehensive initial nursing assessments for all new clients prior to the start of services.
Complete required 30-day reassessments.
Complete required 60-day reassessments.
Complete ongoing reassessments at least every 90 days thereafter or more frequently as required.
Conduct reassessments whenever there is a significant change in the client’s physical, mental, or functional condition.
Develop, review, and update individualized Plans of Care based on assessment findings.
Ensure care plans accurately reflect client needs and authorized services.
Evaluate clients for safety risks within the home environment.
Make recommendations for adaptive equipment, environmental modifications, or additional services when appropriate.
Communicate changes in client condition to the Administrator, physician, case manager, and other appropriate healthcare professionals.
Provide caregiver instruction regarding the client’s care needs and safety precautions.
Monitor the quality and effectiveness of services provided by Personal Care Aides and other direct care staff.
Participate in client case conferences as requested.
Maintain accurate, complete, and timely clinical documentation in accordance with agency policy and South Carolina Medicaid requirements.
Ensure documentation is completed within required timeframes.
Participate in quality assurance, performance improvement, and compliance activities.
Maintain confidentiality in accordance with HIPAA regulations.
Maintain current nursing licensure and complete all required continuing education.
Qualifications
Current, unrestricted Registered Nurse (RN) license in the State of South Carolina.
Graduate of an accredited school of nursing.
Minimum of one (1) year of nursing experience preferred.
Home health, personal care, long-term care, or community health experience preferred.
Knowledge of South Carolina Medicaid and DHEC regulations preferred.
Excellent assessment, documentation, and communication skills.
Ability to work independently and manage multiple client visits.
Valid South Carolina driver’s license.
Reliable transportation.
Current automobile insurance.
Current CPR certification.
Physical Requirements
Frequent driving to client homes.
Ability to lift up to 25 pounds occasionally.
Ability to sit, stand, bend, kneel, and walk for extended periods.
Visual and auditory ability sufficient to assess client condition.
Ability to use electronic documentation systems.

Working Conditions
The RN will routinely travel to clients’ homes within the agency’s service area. Exposure to varying home environments and weather conditions should be expected.
