Job Summary:
Glenwood Care Center is seeking a detail-oriented and knowledgeable MDS Nurse to join our team on a part-time basis. The MDS Nurse plays a critical role in ensuring accurate resident assessments, care planning, and compliance with state and federal regulations within a skilled nursing facility.
Key Responsibilities:
- Complete and coordinate the Minimum Data Set (MDS) assessments in accordance with CMS guidelines
- Ensure timely and accurate submission of MDS to support appropriate reimbursement
- Collaborate with interdisciplinary team (nursing, therapy, dietary, social services) to develop and update individualized care plans
- Monitor and track resident outcomes, ensuring quality measures are met
- Maintain compliance with all federal, state, and facility regulations
- Participate in care plan meetings and communicate resident needs effectively
- Review documentation to ensure accuracy and support coding on MDS assessments
- Assist with audits and survey readiness related to MDS and care planning
What an MDS Nurse Does in a Skilled Nursing Facility:
An MDS Nurse is responsible for assessing residents’ clinical conditions and translating that information into standardized data used for care planning, quality reporting, and Medicare/insurance reimbursement. This role is essential in ensuring residents receive appropriate care while the facility remains compliant and financially accurate.
Qualifications:
- Active California RN license (preferred) or LVN license (required minimum)
- Previous MDS experience in a skilled nursing facility strongly preferred
- Strong understanding of CMS guidelines and reimbursement systems (PDPM)
- Excellent attention to detail and organizational skills
- Ability to work collaboratively with interdisciplinary teams
- Strong communication and documentation skills
