Part-Time Hospice Physician (MD)- Requires Attendance at a monthly 1 hour meeting with Palliative Nurse Practitioners, and one 3 hour interdisciplinary meeting every 2 months. Supervision and guidance for Nurse Practitioners, Approximately 2 hours a month. Serve as backup to the Medical Director as needed (Available for calls from clinical staff for admission approvals, medication authorizations and questions). Core Responsibilities:
- Attendance at a monthly 1 hour meeting with Palliative Nurse Practitioners
- One 3 hour interdisciplinary meeting every 2 months
- Participate in developing and reviewing the hospice plan of care
- Review patient's initial and continued eligibility for hospice services in accordance with applicable state, federal and Unity guidelines
- Be available for on-call consultation with patients, attending physicians and hospice team members regarding a patient's condition
- Make patient home visits as necessary
- Perform such medical and administrative duties as required by applicable Medicare/Medicaid/State regulations or as assigned by Unity's Administrator from time-to time.
- Write medication and treatment orders
- Act as a consultant and liaison to physicians, hospitals, and other healthcare providers to provide information and education about Unity Hospice servicesWho the Job reports to:

Requirements/ Qualifications:
- Active Illinois physician license
- Current DEA certificate
- Driver’s license, insurance and vehicle
