Position Summary
West End is seeking a motivated and highly organized HOPES Case Manager to support justice-involved young adults ages 18–24 through a Department of Labor-funded program. This position will serve youth across Wilmington, Dover, and Georgetown, and will require regular travel between program sites, community partner locations, recruitment events, training locations, and offsite service areas.
The ideal candidate is self-directed, dependable, comfortable working in the community, and skilled at building trust with young adults who may face barriers related to justice involvement, employment, education, housing, transportation, and other life circumstances. This is a hybrid position with a mix of in-office, offsite, and community-based workdays.
Key Responsibilities
The Case Manager will:
- Provide case management services to justice-involved youth and young adults ages 18–24.
- Support participant recruitment through tabling events, canvassing, outreach events, community engagement, and partner referrals.
- Build and maintain relationships with community partners, referral sources, training providers, and local agencies.
- Assist participants with goal setting, employment readiness, training participation, and successful program completion.
- Facilitate, coordinate, and monitor participant trainings and workshops.
- Track participant progress, attendance, outcomes, and supportive service needs.
- Enter accurate and timely data into required data management systems.
- Maintain organized case files and documentation in accordance with program and funder requirements.
- Use motivational interviewing and strengths-based approaches to engage participants.
- Support social media, outreach, and marketing efforts for the program.
- Travel regularly to Wilmington, Dover, and Georgetown to provide services and attend program activities.
- Manage time effectively across multiple locations, deadlines, and participant needs.
- Take initiative in problem-solving, follow-up, and participant engagement.
Qualifications
Qualified candidates should have:
- Experience working with youth, young adults, justice-involved individuals, workforce development programs, case management, social services, or community-based programs.
- Strong communication, documentation, and organizational skills.
- Ability to work independently and manage a hybrid/community-based schedule.
- Comfort with outreach activities such as canvassing, tabling, presentations, and community events.
- Experience using data systems, Microsoft Office, Google Workspace, or similar platforms.
- Ability to maintain professional boundaries and confidentiality.
- Strong time management skills and attention to detail.
- Knowledge of motivational interviewing, trauma-informed care, or strengths-based case management preferred.
- Familiarity with Delaware communities, especially Wilmington, Dover, Georgetown, and Smyrna, preferred.
Preferred Candidate
The ideal candidate will live in or near Smyrna, Delaware, due to the position’s regular travel expectations across New Castle, Kent, and Sussex Counties.
Travel Requirements
This position requires regular local travel throughout Delaware, including Wilmington, Dover, and Georgetown. Reliable transportation, a valid driver’s license, and proof of insurance may be required.
Work Environment
This is a hybrid role that includes a combination of office-based work, community-based outreach, offsite meetings, training locations, and remote administrative work when appropriate.
